Please note that our physicians do have Courtesy Staff Privileges within their facilities. However, our practice is not owned or employed by the Lourdes Health System, and thus, may be considered out of network with your particular plan.
Due to the annual changes made to employer-provided health insurance plan, please be sure to contact our office in advance of EACH visit to confirm that we are still participating in your particular plan
Please contact your primary doctor if you need a referral to see us. If your health insurance plan requires a referral to see us, and you do not have one at the time of your appointment, we have to bill you at the time of your appointment for the services provided or you may reschedule your appointment.
Please contact your primary doctor at least 3 days prior to each of your appointments to confirm your existing referral as well as its expiration date.
